Asthi Dhatu ↔ Bone Metabolism and Physiology
Asthi dhatu's account of bone as a supporting framework and its classification by shape correspond well to modern osteology, and its one exception to the general dosha-dhatu rule -- that bone and vata vary inversely -- correctly captures the association between ageing and bone loss, but the framework has no concept of bone as a living, hormonally regulated tissue.
IN PLAIN LANGUAGE
Classical Ayurvedic physiology describes asthi (bone) as the supporting framework of the body, and uniquely holds that bone and the vata dosha vary in opposite directions -- so bone loss increases vata features and vice versa. This inverse relationship is the reasoning behind treating bone disorders with basti (medicated enemas). It correctly captures that bone loss accompanies ageing, but has no concept of bone as living tissue constantly being broken down and rebuilt, or of vitamin D, calcium, and hormones as specific regulators.
Classical practice offers a correct emphasis on nutrition, weight-bearing activity, and caution about forceful manipulation of fragile bone. Modern medicine offers vitamin D and calcium testing, bone density measurement, and specific treatment for osteoporosis and its risk factors.
WHEN TO SEEK CARE
Bone pain, particularly in an older adult or postmenopausal woman, or any fall resulting in a fracture from minor trauma, needs assessment for osteoporosis and vitamin D deficiency, both very common and both correctable.

- Bone pain with unexplained weight loss, anaemia, or occurring in an older adult (possible malignancy)

- Suspected osteoporosis or osteomalacia
Never do this
Do not perform vigorous manipulation or forceful panchakarma procedures in a patient with established osteoporosis or suspected pathological fracture, since the bone is fragile and vertebral fracture can be precipitated.
