WHEN TO SEEK CARE
A newborn who deteriorates suddenly after feeding starts (possible inborn error), a baby with prolonged jaundice plus lethargy and constipation (possible congenital hypothyroidism), or any family with consanguinity, unexplained infant death, or a child with unusual features needs genetic/metabolic evaluation and newborn screening rather than reassurance.

- Previously well newborn deteriorating after a symptom-free interval, with poor feeding, seizures or encephalopathy (possible inborn error of metabolism)
- Newborn with vomiting, dehydration and shock in the second week of life (possible salt-losing congenital adrenal hyperplasia)
- Congenital heart disease features (cyanosis, breathlessness, absent femoral pulses) in a newborn, especially with Down syndrome
- Developmental regression (loss of previously acquired skills) at any age
- Family history of consanguinity with an affected child, unexplained infant death, or recurrent pregnancy loss
- Any inborn-error presentation (symptom-free interval then deterioration)
- Abnormal or unavailable newborn screening result
- Any structural anomaly found on newborn exam
- Consanguineous couple with a prior affected child or family history of recessive disease
Never do this
Do not rely on clinical suspicion alone for congenital hypothyroidism or other newborn-screening conditions -- by the time clinical features appear, irreversible neurological damage has usually occurred; screening at 48-72 hours with prompt treatment is the only way to prevent it. Do not offer or imply sex-selection services under any framing -- prenatal diagnosis for genetic/congenital conditions is legal in India, but sex determination is prohibited under the PCPNDT Act.
