IN PLAIN LANGUAGE
Classical Ayurveda's description of tamaka shwasa -- episodic breathlessness worse at night and with cold, dust and smoke, relieved by sitting up -- is a genuinely accurate description of asthma, and its dietary and nourishing approach to childhood wasting (karshya) is sound as far as it goes. But a wheezing, wasting or breathless child can also have pneumonia, an inhaled object, tuberculosis, or a heart defect -- conditions needing specific modern tests and treatment, missed for months when treated as 'just a cough' or 'just needs more food.'
Classical medicine offers an accurate clinical picture of asthma and sound general guidance on nourishment, appetite and convalescence -- genuinely useful, and its asthma trigger-avoidance advice is still valid today. Modern medicine offers what classical medicine cannot: counting breaths to diagnose pneumonia, a pulse oximeter to catch hypoxia that looks normal, inhaled bronchodilators for acute asthma, and the systematic workup that finds tuberculosis, congenital heart disease or coeliac disease behind a child who simply isn't growing.
WHEN TO SEEK CARE
Fast breathing, chest indrawing, grunting, stridor at rest, drooling, or bluish lips need same-day assessment; a child who sweats heavily during feeds, tires before finishing, or has repeated chest infections with poor weight gain needs cardiac evaluation; and growth faltering with a TB contact, persistent fever/cough over two weeks, or unexplained lymph nodes needs urgent workup rather than more feeding advice.

- Fast breathing thresholds (60/min or more under 2 months, 50 or more from 2-12 months, 40 or more from 1-5 years) or chest indrawing
- Stridor at rest, drooling, inability to swallow, muffled voice, exhaustion or cyanosis in suspected croup
- Sudden choking/cough/wheeze with unilateral chest signs (possible inhaled foreign body)
- Sweating during feeds, poor weight gain, murmur, or cyanosis worsening on crying (possible congenital heart disease)
- Growth faltering with a TB contact, fever/cough over 2 weeks, or painless lymphadenopathy
- Low oxygen saturation on pulse oximetry regardless of how well the child looks
- Any fast-breathing/chest-indrawing threshold met
- Stridor at rest or signs of upper airway obstruction
- Suspected inhaled foreign body with persistent unilateral wheeze
- Growth faltering not improving despite adequate feeding
- Any cardiac or TB red flag listed above
Never do this
Do not treat fast breathing or chest indrawing with oral/classical measures alone, without antibiotics and referral. Do not examine the throat, lie the child down, or separate a child with suspected croup from their parent -- distress can precipitate complete airway obstruction. Do not manage an acute asthma attack without a bronchodilator. Do not treat growth faltering with nourishing/rasayana measures alone without investigating for TB, congenital heart disease, coeliac disease or other underlying cause.

Warm, vata-kapha-pacifying measures and trigger avoidance (cold, damp, dust, smoke) for ordinary viral cough and for stable asthma between attacks; agni-correction before nourishing measures in stable growth faltering; nutritional and appetite support during convalescence -- alongside, not instead of, antibiotics, bronchodilators, or investigation of an underlying cause.
MODERN MEDICINE SCOPE
Respiratory-rate-based pneumonia diagnosis and antibiotics, pulse oximetry, inhaled bronchodilators and steroids for asthma/croup, bronchoscopy for suspected foreign body, and structured investigation of growth faltering (TB evaluation, coeliac screen, cardiac exam, haemoglobin/stool/urine).
