Galaganda ↔ Endocrinology: Ductless Glands and Endemic Goitre

The classical framework has no endocrine system and no concept of a ductless gland, but galaganda -- correctly sited, and correctly associated with particular regions and their water -- is a genuine observation of endemic iodine-deficiency goitre made without any knowledge of the thyroid or iodine.

IN PLAIN LANGUAGE

Classical Ayurvedic anatomy has no concept of ductless glands releasing chemical messengers into the blood -- the pituitary, thyroid, parathyroids, adrenals and pancreatic islets are not identified as distinct functioning organs, and their regulatory roles are instead distributed across broader concepts like agni (digestive fire) and the vata subtypes. What it does have, in galaganda, is an accurate description of goitre -- correctly located at the front of the neck -- along with the observation that it clusters in particular regions and is linked to the water there. That is an accurate description of endemic iodine-deficiency goitre, made centuries before iodine or the thyroid gland were known.

Classical teaching correctly identified goitre and its geographic clustering, supporting the case for iodised salt rather than against it. Modern medicine adds the mechanism -- the thyroid gland, iodine, and hormone feedback loops -- and with it the ability to detect and treat conditions the classical framework cannot even name, including adrenal crisis from steroid withdrawal, life-threatening low or high blood sugar in diabetes, and hormone-secreting tumours.

WHEN TO SEEK CARE

Anyone on long-term steroid medication -- including unlabelled 'herbal' or fairness creams that may be adulterated with steroids -- should never stop it abruptly; sudden withdrawal can cause a life-threatening adrenal crisis with vomiting, abdominal pain and collapse. A rapidly growing, hard, or fixed thyroid lump, or one with hoarseness or enlarged neck nodes, needs urgent assessment. A treated diabetic who becomes confused, sweaty or aggressive needs sugar immediately -- low blood sugar kills faster than high blood sugar.

Vomiting, abdominal pain, profound hypotension and collapse in anyone who has recently stopped a long-term steroid, including an unlabelled cream or preparation (possible adrenal crisis)
Rapid growth, hardness, fixity, hoarseness or lymph node enlargement in a thyroid lump
Sweating, tremor, confusion, aggression, drowsiness or seizure in a treated diabetic (possible hypoglycaemia)
Thirst, polyuria, weight loss, vomiting, abdominal pain, deep sighing breathing and a smell of acetone on the breath (possible diabetic ketoacidosis)
Cushingoid features -- moon face, central obesity with thin limbs, purple striae, easy bruising -- suggesting undisclosed steroid use
Any suspected adrenal crisis
Any suspicious thyroid lump
Any suspected hypoglycaemia or diabetic ketoacidosis
Any type 1 diabetic considering stopping insulin

Never do this

Never stop a long-term steroid abruptly -- it must be tapered, and abrupt withdrawal can cause fatal adrenal crisis. Never advise a type 1 diabetic to stop insulin -- they have no endogenous insulin, and stopping it causes diabetic ketoacidosis, which kills. Do not repeat the claim that rock salt (sendha lavana) should replace iodised salt for general population use in an iodine-deficient region.
