IN PLAIN LANGUAGE
Before deciding how intensive a treatment should be, classical Ayurveda assesses the patient's strength across ten factors (build, tissue quality, exertion capacity, mental resilience, and more) alongside the disease's strength, and matches treatment intensity to both. Modern medicine does the same thing using named tools like fitness grading before anaesthesia, performance-status scoring before chemotherapy, and frailty assessment before surgery.
Both sciences agree that a treatment which is right for the disease can still be wrong for the patient. Modern medicine offers validated, numeric fitness and frailty scores tested against real outcomes; the classical tenfold exam adds a formally graded assessment of mental resilience (sattva), which most modern pre-treatment scores omit.
WHEN TO SEEK CARE
Before undergoing a major or intensive treatment (surgery, chemotherapy, or an intensive Ayurvedic purificatory procedure), ask whether your overall fitness and reserve have been assessed, not just your diagnosis — this matters especially if you are elderly, frail, malnourished, or have heart or kidney disease.

- A frail, elderly, dehydrated, or cardiac/renal patient being considered for aggressive purificatory therapy (vamana, virechana, or basti)
- Weakness, confusion, palpitations, or fainting during or after purgation- or emesis-based treatment
- Any patient with cardiac disease, renal disease, dehydration, pregnancy, extremes of age, or debility being considered for shodhana-type purificatory therapy
- Electrolyte-disturbance symptoms appearing during or after purificatory therapy
Never do this
Do not perform vamana, virechana, or other strong purificatory therapy in a frail, elderly, dehydrated, cardiac, renal, pregnant, or actively bleeding patient without prior medical assessment and monitoring — the fluid and electrolyte losses from these procedures are real and can cause fatal hypokalaemia or hyponatraemia.
