The upavisha category built an entire pharmaceutical class on the dose-response principle -- the same substance is medicine or poison depending on quantity and preparation -- "centuries before it was stated in Europe" by Paracelsus. But the classical framework lacks any chemical identification of agents, any receptor- or enzyme-level mechanism, and therefore any specific antidote.
IN PLAIN LANGUAGE
Classical Ayurvedic toxicology built an entire category of medicines (upavisha) on a genuinely modern idea: that the same substance can be a poison or a medicine depending purely on how much you take and how it's prepared -- the same principle a European physician named centuries later. Classical texts also described, in their own words, how quickly certain poisons act and spread through the body -- descriptions that match real pharmacological behavior. But they had no way to identify exactly which chemical was involved or to develop a specific antidote for it.
Classical Ayurvedic teaching correctly recognized that dose determines whether something is medicine or poison, and it accurately described how certain poisons behave in the body. Modern medicine adds the ability to identify exactly which chemical is responsible and to use antidotes designed specifically for it -- something no amount of classical description alone can provide.
WHEN TO SEEK CARE
Any suspected poisoning needs prompt medical attention -- do not wait to see if symptoms appear, since many poisons take time to show their full effect even when someone initially looks fine.

- Any patient who looks well shortly after a known or suspected poison exposure (many poisons have a latent, deceptively well-appearing period)
- Any upavisha dravya (vatsanabha, kupeelu, dhattura, gunja, bhallataka, jayapala, ahiphena) taken without correct classical processing (shodhana) or at an uncontrolled dose
- Any suspected poisoning where the specific substance is not confidently identified
- Any adverse reaction to an upavisha dravya
- Any patient with a known toxic exposure who appears well, given how many poisons have a latent period
Never do this
Do not administer an upavisha dravya (vatsanabha, kupeelu, dhattura, gunja, bhallataka, jayapala, ahiphena) without correct classical processing and a controlled, specified dose -- these substances are, by classical definition, poisons at any other preparation or quantity. Do not reassure a patient or family that a poisoning is mild because the patient currently looks well -- many poisons, classical and modern alike, have a latent period before their effects appear. Do not rely on classical description of a poison's qualities as a substitute for chemical identification when the specific substance and its mechanism-specific antidote matter for treatment.
