Modern endocrinology describes hormones, glands and feedback axes in precise biochemical terms; classical Ayurveda has no equivalent framework, though several of the conditions it describes by dosha pattern are, in hindsight, accurately observed endocrine diseases.
IN PLAIN LANGUAGE
Hormones are chemical messengers that control metabolism and growth. Classical Ayurveda has no concept of hormones or glands, though some of the conditions it describes by symptom pattern turn out, in modern terms, to be hormone disorders such as an underactive thyroid.
Modern medicine offers precise blood tests that identify exactly which hormone or gland is at fault, and specific replacement treatment. Ayurveda offers general constitutional support but cannot diagnose or correct a hormone deficiency on its own.
WHEN TO SEEK CARE
Seek urgent medical care for unexplained severe tiredness, weight change, cold intolerance or swelling in the neck, and always if you are on long-term steroids and become unwell or run out of your medication — this can be life-threatening.

- Adrenal crisis in a patient on long-term steroids who becomes acutely unwell or stops steroids abruptly
- Severe, unexplained hypothyroid features such as bradycardia, hypothermia or altered consciousness, suggesting myxoedema coma
- Signs of a pituitary or hormone-secreting tumour, such as visual field loss or a high hormone level with a high trophic hormone level
- Any patient on long-term systemic steroids who becomes unwell, stops steroids abruptly, or faces surgery or major illness needs steroid cover and urgent medical evaluation, not reassurance
- Suspected hypothyroidism or goitre requires thyroid function testing, and in pregnancy or infancy urgent endocrine referral given the risk of irreversible intellectual disability from iodine deficiency
- Any suspected endocrine tumour, indicated by a high hormone level with a high trophic hormone level, requires imaging and specialist referral
Never do this
Do not manage suspected adrenal crisis, myxoedema coma, or a hormone-secreting tumour with dosha-directed measures alone; these require emergency modern medical management.

General constitutional support, such as kapha-reducing diet and lifestyle, alongside confirmed hormone replacement once an endocrine cause has been diagnosed and is being managed medically.

Ayurvedic clinicians who recognise a kapha-type pattern consistent with hypothyroidism should refer for thyroid function testing rather than treating the pattern alone.
