Vicharchika describes dark, thickened, oozing, intensely itchy patches, and Vipadika describes painful cracks in the palms and soles. These line up closely with two very common modern problems: chronic eczema that has thickened and darkened from long-standing itching and scratching, and fissured hand or foot dermatitis, often from repeated wetting, detergents, or standing barefoot on hard or dry surfaces.

- Any fissure or eruption with signs of spreading infection or fever
- No improvement after a reasonable trial (2-4 weeks) of first-line topical care
- Suspected allergic contact dermatitis needing patch testing
- Lesions atypical for eczema (asymmetry, ulceration, non-healing)
Never do this
Do not use undiluted irritant home remedies, unverified caustic pastes, or prolonged unsupervised topical steroid use on fissured or oozing skin, since these can worsen barrier breakdown or mask an infection.
