Pandu ↔ Pediatric Anemia & Blood Disorders
Classical pandu's five types and its recognition of pica (mrittika bhakshana) as a sign of underlying deficiency are genuine, usable clinical observations, but pandu cannot distinguish iron deficiency from thalassemia trait from leukemia -- a distinction on which safe treatment, and sometimes a child's life, depends.

Classical Ayurveda's description of pandu -- pallor, weakness, palpitation, breathlessness on exertion, swelling, poor appetite -- closely matches how anemia actually presents in a child, and its observation that children who eat earth or clay often have this condition anticipated the modern recognition of pica as a sign of iron deficiency. What it cannot do is tell iron-deficiency anemia apart from thalassemia trait, from a haemolytic anemia, or -- most importantly -- from leukemia, conditions that look similar at first but need completely different, sometimes urgent, treatment.
Classical medicine offers accurate descriptive recognition of anemia's presentation, a genuinely useful observation linking pica to deficiency, and sound dietary/appetite support during recovery. Modern medicine offers what classical medicine cannot: a blood count and smear that distinguish iron deficiency from thalassemia trait from leukemia, and the specific treatments -- iron, transfusion with chelation, or cancer chemotherapy -- that only work when matched to the right diagnosis.
WHEN TO SEEK CARE
A pale child with fever, unusual bruising or bleeding, bone pain, or an enlarged liver/spleen/lymph nodes needs a same-day blood count -- this combination is treated as leukemia until proven otherwise. A microcytic anemia that doesn't improve after a few weeks of adequate iron needs a repeat diagnosis, not a higher dose, since it may be thalassemia trait rather than true iron deficiency.

- Pallor with fever, bleeding/bruising, bone pain, or lymphadenopathy/organomegaly (possible acute leukemia)
- Microcytic anemia not responding to adequate iron over a few weeks (possible thalassemia trait or wrong diagnosis)
- Sudden pallor with jaundice and dark urine (possible haemolytic crisis)
- Sudden severe splenic enlargement with rapid haemoglobin fall in a child with sickle cell disease (splenic sequestration)
- Severe anemia with tachycardia, tachypnoea, hepatomegaly or gallop rhythm (high-output cardiac failure)
- Pallor with fever, bleeding, bone pain or organomegaly
- Microcytic anemia not responding to adequate iron
- Any bleeding disorder presentation beyond isolated, otherwise-well petechiae
- Suspected haemolytic crisis or splenic sequestration
Never do this
Do not give iron blindly to every pale child -- in thalassemia trait and the haemolytic anemias the patient is not iron deficient, and prolonged iron supplementation causes harmful overload. Establish the type of anemia with a blood count and smear before treating. Do not treat pallor with fever, bleeding, bone pain or organomegaly as ordinary nutritional anemia -- this combination is acute leukemia until proven otherwise, and delay converts a highly curable disease into a harder one.
